Scheduling Appointments
Patients are seen in the office by appointment only. Please call our office at 408-356-8004 to schedule an appointment.
To save time, download the "Intake Form" (pdf file; 22K) and bring a completed copy to your first appointment with Dr. Cook. (You will need an Adobe® Acrobat reader to view and print out the intake form. If you do not have a reader, you can obtain a free copy through this link.)
If you have an urgent issue we will make every effort to make sure you are seen as soon as possible.

If you are unable to keep an appointment, please telephone the office at least one day in advance to cancel.
After Hours
After hours, Dr. Cook or one of her colleagues will be available for urgent situations or emergencies. Instructions for contacting Dr. Cook, or the doctor on call, are available by calling our main number 408-356-8004.
Telephone Consults
In general, Dr. Cook prefers to meet with her patients face to face. However, for established patients, in cases where the patient prefers to speak by phone, telephone consultations are available by appointment, at the same fee schedule as a comparable office visit.
Patient Confidentiality
Matters of a sensitive nature are apt to be discussed in the office; all communications are strictly confidential. The only exception is when required by law. If you are under the care of another physician for an active problem, please request that your records be sent before your first appointment. The law requires that such a request be made in writing. Also, please bring the names and dosages of all medicines that you are taking, especially if you need a refill. (also see the notice of privacy practices).
