FROM HWY 85
Take Bascom / Los Gatos Blvd. Exit, turn South on Los Gatos Blvd., then turn left onto Samaritan Drive. Dr. Cook’s office is located on the left, 2505 Samaritan Drive, Suite 408 (see campus map below).
FROM SANTA CRUZ
From Highway 17 North, take Lark Ave Exit and go right to Los Gatos Blvd. Make a left onto Los Gatos Blvd., at Samaritan Drive turn right. Dr. Cook’s office is located on the left, 2505 Samaritan Drive, Suite 408 (see campus map below).
